    Sealed Tenders are invited from Agencies/Transport service providers for providing one Indigo/Swift (Non-AC) Car on rental basis w.e.f. 6th July, 2016 to 30th April, 2017 for the D.A. students of Indian Institute of Management Calcutta, Joka, Kolkatta-700104.

    The Tender Documents can be obtained from the Office of the Senior Administrative Officer (Purchase), IIM Calcutta or the Tender Form may also be downloaded from the Institute’s website: www.iimcal.ac.in.

    Agencies/Transport Service Providers are requested to submit their bid document, complete in all respects along with the copies of Trade License & PAN and the cost of Tender Document of Rs.100/- and Earnest Money Deposit (EMD) amounting to Rs.6,000/- by Demand Draft each drawn in favour of Indian Institute of Management Calcutta and payable at Kolkata, and other requisite documents in the Office of the Senior Administrative Officer (Purchase) located in Administrative Building, (Ground floor), Indian Institute of Management Calcutta, D.H.     The Tender will be opened on 14th June, 2016 at 2.15 P.M.at the Office of the Senior Administrative Officer (Purchase) in presence of authorized attending representatives of the bidders.

    IIM Calcutta reserves the right to reject any or all tenders without assigning any reason thereof.

    General Terms & Conditions:

    1. The engagement is from 6th July, 2016 to 30th April, 2017 on all 7 (days) in a week including Saturdays, Sundays and Holidays. Duty hours of the vehicle will be from 8.00 AM to 10.00 PM (14 hours) which is subject to change.
    2. The vehicle will ply inside the campus only and will run not more than 400 Kms per month.
    3. During duty hours, the Vehicle should follow the schedule for parking of the vehicle and should remain under the control of PGP Office/Transport Department, IIMC.
    4. The vehicle is engaged and used solely by the D.A. students of the Institute.
    5. In each case the monthly invoice of the said vehicle should be submitted within 10 (ten) days of the next month along with the duty charts duly signed by the availing students. Service Tax as applicable should be shown separately in your invoice.
    6. While on duty the driver should keep with him the proper & up-to-date records of the vehicle, valid driving license.
    7. The courtesy and good behaviour on the part of the driver is important. Discourteous or careless driver shall be replaced on demand by the Institute.
    8. The Agency is solely responsible for all actions including payment of any type of claims arising out of infringement of rules, regulations, accident or any other unforeseen happening. In case, the vehicle is withdrawn for maintenance /repair/ breakdown, a substitute vehicle should be provided forthwith.
    9. Liability of the Agency: When the Institute engages a vehicle, all liabilities in respect of the vehicle and the driver including accident to the vehicle, injury to driver and the passengers travelling in vehicle shall rest with the Agency. The hiring authority has no responsibility whatsoever and will not entertain any claim in this regard under the provision of the law.
    10. The engagement and employment of driver and payment of wages as per existing provisions of various labour laws or regulations shall be deemed to be breach of this contract if not adhered to.
    11. The Agency shall assign the job of driving of hired vehicles only to qualified, experienced, licensed drivers and also assume full responsibility for the safety and security of the users. The essential spares are to be stored in the vehicle for trouble free driving. IIM Calcutta shall have no direct or indirect liability arising out of negligent, rash and impetuous driving which is an offence under relevant section of IPC and any loss caused to IIM Calcutta will have to be suitably compensated by the Agency.
    12. Vehicle having commercial registration, shall be supplied to IIM Calcutta and taxes etc., due on such vehicles shall be liability of the Agency. The cost of lubricants, repairs, maintenance, taxes, insurance, etc. will be the Agency’s liability.
    13. The vehicle deployed during the contractual period at any point of time should be well maintained and in perfect running condition as per IIM Calcutta’s requirement with proper pollution check and valid pollution certificate.
    14. Driver of the vehicle must be provided and maintain mobile phones. No extra charges would be paid by IIM Calcutta for the same.
    15. In case of break down/servicing/repair, the Agency shall provide alternate vehicle of same model, failing which vehicle shall be hired from any other source/sources at the risk and cost of the Agency.
    16. The maintenance cost, charges of fuel (petrol/diesel), road tax, permit fee, passenger Tax, Border Tax, challans, salary of the driver, the overtime etc. are the responsibility of the Agency and should be paid by the Agency. Insurance: The provided vehicle must be fully and comprehensively insured covering the risk to the driver and all passengers also.
    17. The Institute shall have absolutely no responsibility to pay any compensation for any accident occurred to any person/representative of the Agency during his to and fro journey and or while attending to the services of the Institute and no other charges whatsoever shall be payable by the Institute to the Agency.
    18. The Institute shall have the liberty to enter into similar contract with any other Agency engaged in Car Rental Services parallelly without making any reference.
    19. The rates will be inclusive of all taxes, i.e. cost of petrol/diesel, lubricants, driver’s salary & allowances etc.
    20. Payment will be made on monthly basis on submission of the bill (duly certified by the user and the Transport department) within 30 working days.
    21. No escalation/extra charges will be allowed over & above negotiated rates during the tenure of contract.
    22. The Driver should be courteous to the users, maintain discipline, decorum, well dressed & should always carry a mobile phone with him. In the event of misbehaviour on the part of driver, IIM Calcutta may impose penalty as deemed fit on the Agency.
    23. IIM Calcutta reserves the right to terminate the contract at any time without assigning any reason thereof by serving 15 days notice.
    24. Sub-contracting Not Allowed :
      The successful bidder shall not subcontract, transfer or assign the task to any other Agency without the previous written approval of IIM Calcutta. In case the Agency contravenes this condition, IIM Calcutta shall be entitled to place the contract elsewhere at the cost and risk of the Agency and all expenses borne on this account shall be recovered from him.
    25. Penalties:
      1. In case of break-down of the vehicle, the same to be replaced by other vehicle in good condition immediately (maximum one hour from the time of break-down). Failure on the part of contractor to provide such replacement will attract imposition of penalty on pro-rata basis against each break-down.
      2. IIM Calcutta also reserves the right to impose penalties for unsatisfactory services which may include:
        1. Delayed arrival/report at the designated stop/place.
        2. Misbehaviour with the users.
        3. Violation of instructions given by IIM Calcutta.
        4. Poor quality of cars (both interior & exterior)
        5. The final decision of the extent of penalty leviable on the Agency will rest with the Institute Authority and it is binding on the Agency.
    26. Arbitration:
      All disputes shall be tried to be settled mutually my making references to conditions of contract documents or prevailing local practices etc., but if not settled mutually, shall be referred to arbitration. Such arbitration shall be governed by the provisions of the Indian Arbitration and Conciliation Act, 1996 or as amended from time to time. The venue for such arbitration will be in Kolkata. The award of the Arbitrator shall be final, conclusive and binding on all parties to the contract.

  • Empanelment of Vendors for Supply of Books

    Sub.: Empanelment of Vendors for Supply of Books to PGP (Office) at IIM-Calcutta.

    Indian Institute of Management Calcutta invites open offer of interest for “Empanelment of Vendors for the Supply of Books” on prescribed Format from reputed vendors/distributors/suppliers in India, to its PGP (Office).

    This empanelment will be valid for a period of three years from the date of final approval of empanelled suppliers list and it may be further extendable with written request from the vendors/suppliers, at the sole discretion of the Institute. Interested vendors/distributors/suppliers may submit the applications along with one copy each of the requisite documents to Senior Administrative Officer (Purchase), Indian Institute of Management Calcutta, Joka, Kolkata-700104.

    Last Date for Submission of Application: 12th May, 2016 by 2.00 P.M.

    Instructions for applicants, detailed terms and conditions, application Proforma are as follows:

    Instructions for applicants:

    1. Interested vendors/distributors/suppliers should submit application form in sealed envelopes super-scribing – “Application for Empanelment for the Supply of Books”.
    2. The application should be signed by an authorized signatory of the vendor(s) bearing his/her full name and status, clearly indicated below the signature along with the official seal of the firm.
    3. Incomplete and conditional applications will not be considered.
    4. At any given point of time, if any of the documents furnished by the vendors/suppliers is found to be false, it would be deemed to be a breach of terms of contract making the firm concerned liable for legal action, besides termination of empanelment.
    5. Strict discipline is expected to be maintained with respect to application for empanelment. No canvassing or repeated communication should be made with the PGP (Office). Failing to maintain this discipline, the applicants are liable to be black-listed by the Institute.
    6. The received application(s) after the due date and time will not be considered.
    7. The applications will be scrutinized and shortlisted for empanelment by the Institute committee. The short-listed vendors will be informed by email and/or by regular post for further procedure, as required.
    8. The short-listed vendor(s)/distributor(s)/supplier(s) for empanelment are required to agree to supply as per the Institute’s “Terms & Conditions for Supply of Books (Print)” as stipulated hereunder.

    The empanelment for supply of books (print) will be governed by the following “Terms and Conditions”.

    1. General:
      1. The competent authority of Indian Institute of Management Calcutta reserves the right to approve or reject any or all the vendors, whose decision will be final in all cases in respect of acceptance/rejection/ arbitration.
      2. IIM-Calcutta does not bind to place the purchase order to the approved vendor.
    2. Purchase Orders:
      1. The PGP (Office) will place purchase orders with the empanelled vendor(s) who offers best and prompt services and will try to distribute purchase orders equally among vendors. However, order will be placed on lowest quotation basis.
      2. Supply of books has to be made strictly against the purchase order only.
      3. Sending an acknowledgement of the receipt of purchase order, which is taken as an acceptance of the purchase order, is mandatory, preferably by email.
      4. Any clarification/query regarding the purchase order should be sought from the PGP (Office) within 03(three) days of receipt of the order.
    3. Time-frame for supply:
      1. 20 days -30 days (maximum) – for Indian/ Foreign titles (if available in India).
      2. 3 weeks – 4 weeks, 45 days (maximum) – for Foreign titles (if not available in India).
      3. The Books should be consigned to the Chairperson (PGP), IIM-Calcutta, Joka, Kolkata-700104.
      4. If the requested title(s) is OFP (out of print) or POD (print on demand), sufficient valid supporting documentary proof should accompany the communication seeking any extra time for supply.
      5. In case of titles that are not supplied, a certificate of non-availability of those titles in the market from the publisher/publisher’s distributor/stockiest should be furnished within the due date of supply.
      6. In case there is a delay that is foreseen in supply, the concerned empanelled vendor should send a communication to the Chairperson (PGP) explaining the same and seeking an additional required time to supply the same, at least 07(seven) working days before the supply due date.
      7. If the provided reason for delay is justified with adequate proof e.g., communication from publisher/publisher’s distributor or stockiest, the Chairperson (PGP) may consider extending the supply time as may deem fit.
      8. The supply should be free of freight charges.
      9. Every supply should be accompanied by a delivery challan, clearly bearing the details of the items and titles in supply, their quantity and price. The representatives of vendors/suppliers will be responsible to arrange the books in the distribution room as per requirement of the office
    4. Conditions for cancellations of the released purchase orders:
      1. If the empanelled vendor(s) to whom the order has been placed fails to supply the entire order or any part of the order within the stipulated time, without sending any written communication to the Chairperson (PGP) regarding delay or its (their) inability to supply the ordered title(s), then the empanelled vendor(s) will be charged with liquidated damages at the rate of 2% - 10% of the value of the order not fulfilled, per week, up to four weeks.
      2. Thereafter, the Institute reserves the right to cancel the order and place the empanelled vendor(s) in its blacklist after providing them an opportunity to represent their side.
      3. The decision of accepting supply of cancelled titles is at the sole discretion of the Institute and the decision of the competent authority of the Institute shall be final in this regard.
    5. Edition specifications:
      1. Latest editions of books must be supplied.
      2. By default, paperback editions of books must be supplied.
      3. By default, Indian editions of books must be supplied.
      4. In case of unavailability of paperback and Indian editions, supply of the available editions in lieu of the default is accepted.
    6. Discount:
      Discount for foreign publications and Indian publications should be mentioned separately by the supplier.
    7. Mandatory enclosures with Invoice:
      1. For price verification book seller shall generally produce the publisher’s invoice under which he purchased the book. A copy of the publisher’s/distributor’s invoice/copy of printed catalogue of the order period may be also accepted if above is not applicable.
      2. A currency conversion proof with date.
      3. Every price proof and currency conversion proof should contain seal and authorized signature of the vendor.
    8. Termination of empanelment:
      A vendor’s empanelment may be terminated/dropped/black-listed from the panel of suppliers at the occurrence of any of the following event:
      1. In case of breach of any terms of agreement, or unsatisfactory/inefficient working on the part of the vendor.
      2. If at any time, found that the information provided by the empanelled vendor in any form about publications, services and related matters are incorrect and result in losses in any form to the Institute.
    9. Other Terms and Condition:
      1. The Institute reserves the right to change or modify or amend or substitute any clause(s) in the “Terms and Conditions”, if required, at any time.
      2. No vendor shall have the sole right to supply books/publications notwithstanding the discount rates so decided, the IIM-Calcutta shall have the right to procure books/publications on higher discount rates.
      3. Number of vendors/suppliers/publishers etc. for empanelment will be fixed by IIM-Calcutta.
      4. The book supplier shall deliver the books/documents to PGP (Office) premises at his own expenses. No freight for to and fro transportation shall be paid by the PGP (Office). If freight for to and fro transportation etc. is applicable, it should be communicated in writing in advance.
      5. In case of urgency, the vendors need to deliver the books under a very short notice. After placement of order (full quantity), in case of failure by the vendor to deliver the books within stipulated time, the vendor will be blacklisted by the competent authority of the Institute.
      6. The book supplier has to supply the new books in good condition and shall be responsible for misprint, defected piece from publisher, and any damaged books due to any reasons during transportation, etc. If any book/s found damaged, the recovery for the same will be adjusted from outstanding payment/pending bills of the book supplier. Penalty will be imposed for failure of delivery on stipulated time.
      7. In case of books of international edition, quotation in foreign currency (US$/British Pound) will be acceptable. Excess copies of book(s) will have to be taken back from the Institute, at the own responsibility of the vendor. The Institute will have no liability for these returnable books. Billing will be done on the basis of final challan with receipted quantities of books.
      8. Separate quotation is to be submitted for each titled book in sealed envelope and addressed to the Chairperson (PGP), IIM Calcutta. Name of the titled book should be mentioned on the envelope.
      9. The book supplier has to submit the bill only on the basis of receipted. On delay of any payment, no claim of interest or any charge will be entertained by the PGP (Office).
      10. All disputes and differences arising out or concerning the work shall be subject to the sole arbitration of Director, IIM-Calcutta or his nominee. The decision of the arbitration shall be final & binding on both the parties. The empanelment will be interpreted under Indian Laws and disputes adjustable within the jurisdiction of court of law of IIM Calcutta.
      11. Appeal against any terms & conditions shall, however, lie with the Director, IIM Calcutta whose decision shall be final. The dispute, if any, shall be subject to the jurisdiction of Courts at Kolkata. Any other jurisdiction mentioned in the quotations or invoices of the book supplier shall be invalid and shall have no legal sanctity.

      13.     Interested Creative Advertising Agencies with INS Accreditation meeting the eligibility criteria may submit their EOI for empanelment complete in all respect to Senior Administrative Officer (Purchase & Advertisement), Indian Institute of Management Calcutta, Joka, D. H. Road, Kolkata- 700 104.

    Agencies, should apply with a non-refundable application fee of Rs.500/-(Rupees five hundred) only, payable in the form of a Demand Draft from any scheduled commercial Bank favouring ‘Indian Institute of Management Calcutta’ payable at Kolkata.     Senior Administrative Officer (Purchase & Advertisement)

    1. Eligibility Criteria:
      The following are the eligibility criteria for empanelment of Advertising Agencies.
      The Advertising Agency should have –
      1. Accreditation of Indian Newspaper Society(INS) for Press Advertisement (updated documentary evidence to this effect should be enclosed)
      2. The Agency should have office in Kolkata and with state-of-the-art infrastructure and manpower/ creative personnel/department (enclosed organizational structure).
      3. PAN India-Presence with offices at least in major metros so as to enable prompt delivery of material for country-wide publication related work.
      4. Resources to handle full-fledged multi-lingual publicity campaigns with high standard of Creativity in advertising befitting the expectations of a premier professional Institute.
      5. Proficiency and proof reading facilities in major Indian languages.
      6. Minimum 5(five) years experience in advertising with experience of work undertaken for Government/Public Sector Undertaking/Autonomous Body and Educational Institutes like IIMs, IITs.
      7. Annual turnover preferably around Rs.6(six) Crores (Documentary evidence (Audited Annual Report of the last financial years of 2012-13, 2013-14 and 2014-15 to this effect should be enclosed.
      8. The Advertising Agency applying for empanelment should not have been blacklisted by any organization at any point of time. (An undertaking in this regard should be submitted duly signed by the authorized person of the Organization).
      9. The Agency should have Service Tax Registration (documentary evidence to be submitted).
      10. The Advertising Agency should have adequate infrastructure for timely publication.
    2. Scope of Work:

      Institute's advertising works ranges from classified advertisements, tender advertisements to full-fledged advertisements for student's admissions, recruitment, tender etc. They are expected to be released in major newspapers, magazines and Journals all over the country and abroad at short notice.

      To cater to such requirements, Advertising Agency is required to have good infrastructure and have adequate experience in the field. These should include creation of artwork, concept, design, editing & release of advertisements in Newspapers, Magazines & Journals etc.

    3. Terms & Conditions:
      1. The empanelment is valid for a period of two years from the date of approval of the competent authority which may be extended for a further period of one year subject to satisfactory performance and verification of documents as asked for by the authority of IIM Calcutta.
      2. The Institute will have the right to drop any Agency from the empanelled list without assigning any reason whatsoever. Institute also reserves the right to modify the Terms and Conditions for empanelled Agencies.
      3. The empanelled Advertising Agency is expected to maintain high level of professional ethics and will not act in any manner, which is detrimental to Institute's interest. Agency will maintain confidentiality on matters disclosed till proper instruction is issued for publication. IIM Calcutta reserves the right to impose penalty in case of any violation of the above.
      4. The Agency should be able to execute all kinds of Advertising Assignments.
      5. Selection of artwork will be entirely on Institute's discretion and IIM Calcutta will not pay charges for submission of artwork.
      6. IIM Calcutta reserves the right to make necessary modification to the selected artwork, concept etc. and the concerned Advertising Agency will be required to carry out the modifications suggested in the artwork.
      7. It shall be the responsibility of the agency to release all the Advertisements in the minimum possible space in the newspapers.
      8. All the Advertisements concerning the Institute shall be issued with the prior approval of the IIM Calcutta.
      9. The representative(s) of the Agency will collect the matter personally from the Purchase & Advertisement Department and the Agency will arrange to get it published in the desired media, on desired date(s) and on the defined page(s).
      10. If the Advertisement is misprinted or published differently from the approved one, the Agency will publish corrigendum/correct Advertisement etc. thereof at their own cost at the earliest (within a week).
      11. Translation of the Advertisement matter whenever required will be done by the Agency free of cost and the Agency will be responsible for the same.
      12. Payment shall be made within 30(thirty) days after receipt of the final bill, duly supported by all the newspaper clippings (in original).
      13. No extra payment shall be made for conceptualization, creation, designing and development of Advertisement issued through Newspapers/Magazines/Journals.
      14. The Institute shall have the discretion to give work to any empanelled Agency selected through tendering process and the Agency will have no right or claim for getting the work.
      15. The performance of the Agency shall be regularly reviewed on the key parameters of creativity, initiative, competency and response time and participation in the tendering process. In the event that Agency fails to meet our requirements, we shall be constrained to terminate the empanelment by serving one month's prior notice.
      16. The IIM Calcutta reserves the right to empanel any other Advertising Agency or cancel empanelment of any agency without assigning any reason by serving one month's notice even before expiry of the period of empanelment.
      17. Empanelled Agency will not be allowed to engage any other Sub-agency to execute the assigned work/job.
      18. The Institute does not guarantee any minimum business or assignment which will depend on the requirements, financial resources available and your performance.
      19. In case of any dispute arising out of the terms and conditions of contract or assignment, the matter shall be referred to the Sole Arbitrator to be appointed by the Director, IIM Calcutta and the award of the Sole Arbitrator will be binding on both the parties. Further, any legal dispute arising out of any breach of contract pertaining to this empanelment shall be settled in the court of competent jurisdiction located within the local limits of Kolkata, West Bengal.
      20. IIM Calcutta being a Premier Educational Institute, many of the requirements could be of emergent in nature. The Advertising Agency has to respond to such requirements at short-notice despite holidays/beyond office hours.
      21. It is clearly understood by the parties that no financial liability of any type is created by issuance of the letter of empanelment.
      22. Applications received after due date & time or without necessary documents will be rejected.

      1. Brief profile of the Advertising Agency and experience in the advertising field. (At least 05 years of experience).
      2. Agency Structure (whether partnership firm/sole proprietor/a limited company etc.).

      4. Profile of its managerial team handling advertising work, their qualifications and experience.
      5. Details of their Accreditation with Indian Newspaper Society (INS) along with the date of accreditation (Full Accreditation).
      6. Details of members with other professional organizations/associations and International assignments, if any.
      7. Details of experiencein dealing with Government Institutions, Government of India Undertakings, Autonomous Bodies and professional Institutes like IIMs, IITs.
      8. List of Clients.
      9. Samples of creative work undertaken for major organizations in the previous year.
      10. Annual turnover (minimum Rs.6(six) crores for previous three financial years of the Agency  duly certified by a Chartered Accountant.

    Indian Institute of Management Calcutta, which is an autonomous body under the aegis of Ministry of Human Resources Development, Government of India desires to empanel well qualified Printers experienced in conceptualizing, designing, printing of documents etc. having sound back-up for excellent designing, scanning, planning etc. of publication of various descriptions/literature for black & white and multi-colour jobs etc. The printing and designing jobs broadly include designing and printing of In-house Magazines, Newsletters, Bulletins, Guidelines, different types of Brochure for different programmes, Annual Report, Posters, Pamphlets, Visiting Card, Security Printing of Grade Sheet, Diploma Certificate, Greeting Card, Desk Calendar, Special types of files, Folders, etc. on the basis of modern facilities standards available in the industry.

    The detailed terms & conditions for empanelment of Printers with IIM-Calcutta are given in Annexure – “A”. Interested Printers may apply for empanelment to IIM-Calcutta in the prescribed Proforma given at Annexure - “B”.     Terms & Conditions for Empanelment:

    1. The empanelment is valid for a period of two years from the date of approval of the competent authority which may be extended for a further period of one year subject to satisfactory performance and verification of documents as asked for by the authority of IIM Calcutta.
    2. Printers experienced in conceptualizing, designing, printing of documents etc. having sound back-up for excellent designing, scanning, planning etc. of publication of various descriptions/literature for black & white and multi-colour jobs, security printing of grade sheet, diploma certificate are eligible to apply. The printing and designing jobs broadly include designing, photography as and when necessary, printing of In-house Magazines, Newsletters, Bulletins, Guidelines, different types of Brochures for different programmes, Annual Report, Posters, Pamphlets, Visiting Card, Greeting Card, Desk Calendar, Special types of files, Folders, etc. on the basis of modern facilities and standards available in the industry.
    3. The Printer should be in the profession of printing work at least for the last 3 financial years.
    4. The Printer should be on the approved panel of Central Govt.//State Govt./PSUs./IITs/IIMS/NITs.
    5. The Printer should have registration with state & local authorities for undertaking the profession (copies of all such registrations may be enclosed).
    6. The Printer should have at least one single colour & colour offset machine, In-house stitching & binding unit & screen-printing unit.
    7. The rights to postpone/cancel/terminate the empanelment or its process at any stage is reserved by the competent authority of IIM-Calcutta.
    8. Application of the intending Printers can also rejected at the sole discretion of IIM-Calcutta, if any application found incomplete or information provided by the Printer is not in the prescribed format and /or is inconsistent and inaccurate.
    9. IIM-Calcutta reserves the right to remove any Printer from the approved empanelled list for designing and printing work etc. for any administrative reason.
    10. The Printers should have full-fledged unit of their own for visual design facility, preparation of art work and translation in English, Hindi etc. proof reading, designing, composing, photograph, printing etc.
    11. In the event of the delivery of any defective work/materials, owing to urgency or for any other reasons, IIM-Calcutta shall reserve the right to deduct such suitable sum subject to maximum of 10% of order value from any payment due to the Printer.
    12. Tender Enquiry for the printing work will be sent to the empanelled Printers through e-mail or by-hand or speed post/registered post-courier etc.
    13. The empanelment can be terminated by either party prior to the duration of 24 months by serving one month’s notice to the other party. In case of termination, the entire materials whatsoever pending with Printer will liable to be returned to IIM-Calcutta.
    14. A schedule for completing the work and delivery of printed copies will be drawn by the competent authority of IIM Calcutta and will be intimated to the Printer concerned while placing the work-order for the work. The Printer will strictly follow the schedule for completing the work and delivery of items, failing which the name of Printer will be de-listed from the empanelment by IIM-Calcutta authority.
    15. The Printer applying for empanelment should furnish all the particulars as per Appendix – “B”.
    16. The date of empanelled printers will be declared at the discretion of IIM-Calcutta which could be intimated through any means of communication as mentioned above.
    17. All the items to be received from the Printer will be checked by the concerned department in order to ensure specifications/quality as per samples approved by the concerned department.
    18. Payment will be released on submission of bill supported with the copy of the work order of the authorized offers of IIM-Calcutta and upon such certification to the effect that the work got printed from the Printers are as per approved specifications.
    19. Additional/modified/revised terms & conditions can also be made applicable to the empanelled Printers including those who are being awarded any work by IIM-Calcutta at any stage as per requirements of IIM-Calcutta, if felt necessary.
    20. It will be the responsibility of the Printer to return all materials concerned like manuscript, art-work, photographs, CDs/DVDs etc. after completion of the work, failing which the costs of the items will be recovered from the Printer.
    21. The empanelled Printer should have adequate arrangements for packing and dispatching the documents, if so required.
    22. Printers must have office and press in Kolkata.
    23. The Printer should not have been placed in defaulter category by any Government department/institutions. If not, please submit a self-attested certificate to this effect.
    24. No terms & conditions from the applicants will be entertained.
    25. In case of any dispute arising out of the terms and conditions of contract or assignment, the matter shall be referred to the Sole Arbitrator to be appointed by the Director, IIM Calcutta and the award of the Sole Arbitrator will be binding on both the parties.     for

    Empanelment of Agencies for Hiring of Vehicles

    Indian Institute of Management Calcutta invites ‘sealed bids’ from reputed agencies having experience of similar work contract for the work, “Empanelment of Agencies for hiring of vehicles” fulfilling the criteria as laid down in Technical Bid format at Annexure- II.

    Agencies may submit their bid document, complete in all respects along with the cost of Tender Document of Rs.500/- and Earnest Money Deposit (EMD) amounting to Rs.50,000/- by Demand Draft each drawn in favour of Indian Institute of Management Calcuttaand payable at Kolkata, and other requisite documents in the Office of the Senior Administrative Officer (Purchase) located in Administrative Building, (Ground floor), Indian Institute of Management Calcutta, D.H. Road, Joka, Kolkatta-700104 by 2.00 P.M. on 11thMay, 2016.

    Tenders are to be submitted in twoseparate envelopes. Envelope-1 super-scribing ‘EMD’ should contain the cost of tender documents (Rs.500/-) and EMD (Rs.50,000/-) in the form of Demand Draft drawn in favour of Indian Institute of Management Calcutta and payable at Kolkata. Envelope-2 super-scribing‘Technical Bid’ should contain Technical Bid in the specified format in Annexure II duly filled-in along with the tender documentduly filled-in, signed and sealed in all pages.The above two envelopes are to be put in a bigger envelope super-scribing as “Empanelment of Agencies for Hiring of Vehicles at IIM Calcutta”.

    1. Scope of Work :

      The scope of work covers hiring of vehicles of different category of vehicles for IIM Calcutta to travel within Kolkata & suburbs as and when required and as per terms & conditions of the tender. The estimated business value of the work for empanelment is Rs.25 lakhs (approx.) per annum.

      The Agency shall provide AC/Non-AC vehicles duly registered in Kolkata and must not be more than 5 years old at any point of time and comply with emission norm of Bharat Stage III / IV (Euro III / IV) of the following categories:
      Tata Indigo/Equivalent, Swift Dzire/Equivalentand Innova/Equivalent.

    2. Last Date for Submission of Tender :

      The tender documents duly completed in all respect must reach to the Senior Administrative Officer (Purchase), Indian Institute of Management Calcutta, Joka, Kolkata- 700104 on or before 2.00p.m. on 11thMay, 2016.If the last date of receiving/opening of the tenders coincide with a holiday, then the next working day shall be the receiving/opening date.

    3. Bid Validity:

      All the bids must be valid for a period of ninety(90) days from the date of tender opening.

    4. Earnest Money Deposit:

      The application must be accompanied with an EMD of Rs.50,000/- in the form DD in favour of Indian Institute of Management Calcutta and payable at Kolkata. The EMD of empanelled Agencies will be kept as Security Deposit till the period of empanelment plus three months.EMD of rest Agencies will be released within 15 days from the date of finalisation of empanelment without any interest.

    5. Duration of Empanelment:

      At the initial stage, the empanelment for hiring of vehicles shall be awarded for a period three years, extendable for a further period of one year based on the satisfactory performance of the Agency and acceptance of the same terms & conditions. However, it will be obligatory on the part of Agencyto continue to work at the rates prevailing on the last date of the empanelment even beyond empanelment period for at least 3 (three) months or till the new empanelment is finalized, whichever is earlier.

    6. Terms and Conditions:
      1. Conditional tenders will not be accepted and will be summarily rejected.
      2. The IIM-Calcutta's offer of the contract to the agency so empanelled would need to by accepted within 7(seven) days from the date of issue of the offer. Failure to accept the offer within this period will result in cancellation of the offer.
      3. The agencies should quote one price for each type of vehicle specified in Annexure- III. The price bid not submitted in the format specified in the tender document will not be accepted & the application will be rejected.
      4. It may please be noted that while booking the car, the car agency has to honour it'ss commitment and provide the cars to IIM-Calcutta. In case, the agency is not in a position to provide the car, it will be viewed seriously and the contract with such Agencywill be terminated immediately due to the agency not able to provide the car to IIM-Calcutta.
      5. The Agency shall have all necessary mandatory approvals/certificates/registration, etc.All the vehicles to be supplied to IIM-Calcutta will be registered as Commercial/Tourist vehicles and shall have comprehensive insurance cover with adequate clause to cover the passenger for injuries/death and all matters related to insurance will be settled by the agency. In no case vehicle, which is not registered for commercial purpose, shall be supplied to IIM-Calcutta.
      6. In case, any information / document is (are) found to be false or incorrect at a larger stage also it shall invite immediate disqualification/termination of the agency contract from empanelment. EMD in such cases will be forfeited. The original document may be verified / scrutinized during the tendering process.
      7. The vehicle should be in good condition and should be well maintained, properly cleaned inside and outside and in good hygienic condition.
      8. The drivers shall have a valid driving license and should be well behaved, in proper uniform with identity card indicating their name and name of the Agency while on duty and shall carry common mobile phone (at the cost of Agency) with them. The courtesy and good behaviour on the part of the driver is important. Discourteous or careless driver shall be replaced on demand by the Institute.
      9. The drivers shall not enter the office premises / floor of IIM-Calcutta for any other reasons except for attending to the services of the faculty members/officers/staff to whom the agency'ss car is allotted for services.
      10. Any damages/repairs of the vehicle shall be borne by the agency.
      11. Tax at source, shall be deducted at the rates applicable from time to time from the bill amounts payable to the agency.
      12. The IIM-Calcutta reserves the right to terminate the contract of empanelment at anytime without any notice or assigning any reason therefore. Dues on pro-rata/mileage basis will be settled till the date of terminated contract and the decision of the IIM-Calcutta in this regard is final.
      13. In case, the vehicle is withdrawn for maintenance, repairs, breakdown, etc., a substitute of same variant vehicle in very good condition should be provided.
      14. Payment Terms- Monthly bills shall be submitted to the authority specified in the contract. Parking charges shall be reimbursed on actual basis subject to the production of parking receipts (original).
      15. The successful bidder/s shall accept the offer along with the terms & conditions stipulated by the IIM-Calcutta in writing.
      16. In the event of the IIM-Calcutta requiring more number of vehicles, the Agency should be in a position to meet the additional requirement and prepared to supply the cars at the rates quoted now. In the special occasion, the Agency needs to provide specific car as required by the Institute.
      17. The Agency shall provide the vehicle at IIM Calcutta, Joka as per requirement or at any other place intimated to the agency for travel in Kolkata or its suburbs. The maximum dead mileage allowed towards to & fro journey from garage to the place of duty will be 10 (ten) Kms or actual whichever is less.
      18. A Log Book/Duty Slip as per the standard of industry has to be maintained for every vehicle/duty provided by the vehicle hire agency recording the opening/closing kms., time and location of trip. It shall be the responsibility of the hiring agency to obtain the signatures of the guest/passenger at the end of every trip. Such documents must be attached to every bill tendered for settlement.
      19. The vehicles are required as per necessity from the date of issuance of the work order for empanelment. The contract vehicles are to be used official(s) of the Institute and Institute works only.
      20. The duty hours and kilo-meters will be calculated from the reporting time to releasing time of the vehicle on each day. The Bill to be prepared on the basis of engagement of the car Day &Time/Km/Hr. figures written in the reportrelease column of the Duty Slip. The payment of hired vehicles will be made on monthly basis.
      21. The Log Book or Duty Slip is to be maintained by the agency as per proforma to bespecified by the Transport Department, which is to be acknowledged by the controllingofficer. In case of loss of the said Log-Book or Duty Slip, the controlling officer'ss decisionregarding payment will be final.
      22. The Agency is solely responsible for all actions including payment of any type of claimsarising out of infringement of rules, regulations, accident or any other unforeseenhappening. In case, the vehicle is withdrawn for maintenance /repair/ breakdown, asubstitute vehicle should be provided forthwith.
      23. The controlling officer of this Institute to whom the vehicle shall essentially report isempowered to return the vehicle, if he feels that it is not worth traveling and no payment willbe made for the said day. Moreover, no payment will be made on the day, if theAgency fails to render service in time for the day.
      24. In case of emergency i.e. breakdown en-route, Agency shall have to arrange for alternatevehicle for escorting persons and materials.
      25. Liability of the Agency: When the Institute engages vehicles, all liabilities in respect of the vehicles and the drivers including accident to the vehicle, injury to driver and the passengers travelling in vehicle or to pedestrians, theft and other passing vehicles shall rest with the agency.The hiring authority has noresponsibility whatsoever and will not entertain any claim in this regard under the provisionof the law.
      26. The engagement and employment of drivers and payment of wages to them as per existingprovisions of various labour laws or regulations shall be deemed to be breach of thiscontract if not adhered to.
      27. The agency shall assign the job of driving of hired vehicles only to qualified,experienced, licensed drivers and also assume full responsibility for the safety and securityof the users. The essential spares are to be stored in the vehicle for trouble free driving. IIM Calcutta shall have no direct or indirect liability arising out of negligent, rash andimpetuous driving which is an offence under relevant section of IPC and any loss caused toIIM Calcutta will have to be suitably compensated by the agency.
      28. The cost of lubricants, repairs,maintenance, taxes, insurance, etc. will be the agency'ss liability.
      29. IIM Calcutta will reimburse toll tax, parking charge etc., wherever incurred onsubmission of original receipt.
      30. The vehicles deployed during the contractual period at any point of time should be wellmaintained and in perfect running condition as per IIM Calcutta'ss requirement with properpollution check and valid pollution certificate.
      31. The vehicle shall always be provided with decent upholstery, clean seat covers, comfortableseat cushions and other basic fittings/accessories like radio cassette player etc. formaximum comfort of passengers.
      32. Drivers of vehicles must be provided and maintain mobile phones. No extra charges wouldbe paid by IIM Calcutta for the same.
      33. The Institute shall have absolutely no responsibility to pay any compensation for any accident occurred to any person/representative of the Agency during his/their to and fro journey and or while attending to the services of the Institute and no other charges whatsoever shall be payable by the Institute to the Agency.
      34. The Institute shall have the liberty to enter into similar contract of empanelment with any other agencyengaged in vehicle hiring parallel services without making any reference.
      35. Rates should be in Indian Rupees only. Rates quoted must be firm and final and shall remain constant throughout the period of the empanelment/extended period of empanelment and shall not be subject to any upward revisions whatsoever. Rates should include all charges applicable for the services being offered. Service Tax shall be paid extra as per the government orders from time to time. Parking charges and toll tax will also be paid extra on production of proof.
      36. During the period of empanelment/extended empanelment, if the rates of fuel go up on account of the policies of the government, IIM-Calcutta may consider upward revision of the hiring charges to the tune such hike in fuel charges effect the operating costs. IIM-Calcutta'ss decision will be final and binding in this regard. The empanelment of the agency/agencies refusing to accept IIM-Calcutta'ss decision will be cancelled and Security Deposit will be forfeited.
    7. Special Conditions of the Contract :
      1. The rates fixed by IIM Calcutta (Annexure-III) will be inclusive of all taxes and also include cost of petrol/diesel, lubricants, driver'ss salary & allowances etc.
      2. The official(s) of the agency responsible for the operations of vehicle should be available round the clock on his own direct telephone (office as well as residence) so as to respond to the call for vehicle in emergent case. The Agency should be in a position to supply vehicles on short notice as & when required.
      3. The Driver on duty should report to the Officer-In-charge, Transport Department, IIM Calcutta before starting journey unless otherwise instructed.
      4. Payment will be made on monthly basis on submission of the bill duly certified by the user along with duty slip.
    8. Force Majeure: Neither the agency nor the Institute shall be liable to the other, for any delay in or failure of their respective obligations under this control caused by occurrences beyond the control of either party because of fire, floods, acts of God, acts of public enemy, wars, riots, strikes, lockouts, sabotage any law statute or ordinance order actions or regulations of the Government or any compliance there in similar to the above. Either party shall promptly notify the other of his commencement and cessation of such contingency and prove that such is beyond the controls and effects the implementation of this contract adversely.
    9. Duties andResponsibilities of Agency :
      1. The Agency shall provide West Bengal registered vehicles (Petrol/Diesel/CNG) of Model 2011 or later as per our requirement with well behaved / skilled drivers in proper uniform having knowledge of routes & minor repairs of cars and valid driving licenses. All the cars shall carry First Aid Box, Stepney and Toolbox.
      2. In case of breakdown the Agency shall provide alternate vehicle of same category failing which the touring officials/guests will be allowed to hire any vehicle and complete the journey and the amount thus incurred shall be recovered from the bills of the agency.
      3. All kinds of repairs / maintenance cost, charges of petrol, oil, lubricant, fee towards licenses/ registration taxes such as road tax, permit fee etc., challans, salary/overtime of the driver, insurance premium etc. are the responsibility of the agency and shall be borne by the agency all along. IIM Calcutta will reimburse toll tax, parking charge and state passenger tax wherever incurred on submission of original receipt. The driver is to be provided with petty cash for the purpose.
      4. The drivers should strictly follow all the Traffic Rules and Regulations as prescribed by the Govt. authorities. The Institute will not be responsible for the reimbursement of any charges, charged by Govt. authorities for violation of any Traffic Rules and Regulations.
      5. The Agency must ensure that drivers are not under the influence of alcohol or other intoxications while performing their duties for the Institute. Smoking is strictly prohibited inside vehicles/Institute.
      6. The vehicle on hire shall be available at all time including Saturdays, Sundays and public holidays as per requirement of the Institute.
      7. The Agency shall abide by all statutory acts, motor vehicles act, labour laws and regulations act as they are applicable to his establishment/workmen.
      8. In response to a requisition for a vehicle, agency needs to send a S.M.S.(vehicle No., driver's name with mobile phone number etc.,) to the concerned faculty/officer/staff/guest/participant with a copy to the Transport Department, preferably 12 hours in advance. It is mandatory for the agency to reply through e-mail against any requisition for a vehicle.
    10. Statutory requirements:

      The vehicles sent to IIM-Calcutta on requisition must have all relevant documents like registration Book/Driving license/Insurance/Road tax Receipt/Permit for Passenger Taxi/Pollution certification etc.. The vehicles should be licensed and shall have valid permits for plying in Kolkata and suburbs. The vehicle should conform to all Government rules and regulation being in force from time to time.

    11. Meter Tampering: Agencies would ensure proper sealing of milometer. IIM-Calcutta reserves the right to get the meter calibrated or checked at any time at its sole discretion. Tampering of meter reading and misbehaviour of driver shall be viewed seriously, leading to even cancellation/termination of empanelment and forfeiture of security deposit. In the event of any error/fault in the meter being noticed, the bill for the journey undertaken (including those undertaken earlier) would be adjusted, besides any other penal action.
    12. Sub-contracting Not Allowed :

      The successful service providers shall not sub-contract, transfer or assign the task to any other agency. In case, the agency contravenes this condition, IIM Calcutta shall be entitled to place the contract elsewhere at the cost and risk of the agency and all expenses borne on this account shall be recovered from him. Any claim by the third party on behalf of the Agency will not be entertained from the Institute'ss end.

    13. Penalties :In case of failure to provide vehicle after confirm booking, late reporting (beyond 30 minutes) at the designated place, misbehaviour with the user, improper condition of vehicle interior/exterior, the competent authority of IIMC will have the right to impose financial penalty 5 per cent of the annual bill which will be deducted from the bill submitted by the agency.
    14. Payment :The Payment terms are as follows:
      Payment terms:-
      1. Payment will be made only for total km. run by the car for IIMC on official duty with admissible charges. The minimum charges 50 Kms or 5 hours whichever is higher will be accepted for a trip.
      2. If reporting & releasing at IIMC campus, 10 Kms.(5 Kms.+5 Kms.) will be given extra in addition to actual kms. distance run by the car. 5 Km additional mileage will be allowed in case the duty commences from the IIMC campus for onward journey and similar 5 km from IIMC to the garage of the Agency.
      3. The payment will be made only for those Duty Slips which have been signed by the faculty member/officer/staff/guest using the vehicle. It will be the responsibility of the driver to get the Duty Slips signed by the faculty member/officer/staff/guest on completion of duty. No payment will be made for unsigned duty slips. Duty Slip should be completed in all respect i.e. Time and Odometer Reading at the time of commencement of journey from the Institute/or reporting point. Time and Odometer Reading at the time of journey at Institute/destination, total run of vehicle and places visited for work in-route (if any).
      4. The pre-receipted bill shall be submitted in duplicate of hired vehicles, duly supported by the Duty Slip properly signed by the user/authorized person, and requisite documents {receipt(s) of payment of all taxes, parking fees} on monthly basis. Payment to the agency will be made after deduction of TDS.
      5. No claim in respect of under-payment to the agency shall be considered valid or shall beentertained unless a claim in writing is made within three months from the date on which payment of the original claim thereto was made. Any claim for such under-payment not received within the stipulated three months period shall be liable to be summarily rejected byIIM Calcutta.IIM Calcutta will have the right to recover any over payment which might have been made to the agency by IIM Calcutta through inadvertence, error, etc., or any cause whatsoever from their bills and/or from the security deposit or any other amounts due to him.
    15. Illegal Gratification :

      Any bribe, commission, gift or advantage given/promised or offered by or on behalf of theAgency or his partner, agent or servant or any one of his or on their behalf to any employees(s) of IIM Calcutta  or to any person on his or on their behalf in relation to the obtaining orthe executing of this or any other contract with IIM Calcutta shall entitle IIM Calcutta to rescind all contracts with him in addition to any criminal liability the agency may incur. The Agency shall notlend to or borrow from or have or enter into any monetary dealings or transactions either directly or indirectly with any employee(s) of IIM Calcutta and, if he shall do so, IIM Calcutta shall be entitled forthwith to rescind his all contracts with IIM Calcutta. Any question or dispute as to the commissions of any offense or compensation payable to IIM Calcutta under this clause shall be settled by IIM Calcutta insuch a manner as it considers fit, sufficient and decision in this regard shall be final and conclusive.The Agency shall forbid and take all possible steps within his power to prevent all labourer and other persons employed by him from demanding or receiving from any person other than the Agency himself or his agents any remuneration or gratuity whatsoever.

    16. Bid Evaluation:
      1. Envelope-1 is the first envelope to be opened on the specified date and time in the presence of bidder'ss representatives (one from each bidder). Envelope-2 of only those bidders whose EMD is in order shall be opened in the same session. Bids not accompanying with the prescribed EMD will not be considered.
      2. The contents of Envelope- 2 (Technical Bid) shall be evaluated by the Institute'ss Committee. The Committee will scrutinize the Technical Bids specified in the tender document and may call for additional information from the bidders or may visit the bidders's offices for verification. Additional information, if called for, must be submitted in the time period given by the Committee, failing which the bid shall be rejected.
      3. IIM-Calcutta shall have a panel of minimum four service providers for empanelment  after assessing the Technical bids along with other terms and conditions.
      4. The Agency who will qualify in the Technical Bid and will accept the ratesfor different vehicles and other related charges as per Annexure-III fixed by IIMC will be considered for empanelment.
      5. Any attempt by any Agency to bring pressure of any kind, may disqualify the bidder for the present tender and the bidder may be liable to be debarred from three years from bidding for future IIM-Calcutta tender besides forfeiting the EMD.
      6. The IIM-Calcutta reserves the right to reject any or all the bids or cancel the Tender, if number of agencies available to be empanelled at the rate fixed by IIM- Calcutta is insufficient to meet the requirement without assigning any reason whatsoever. The IIM-Calcutta also reserves the right to reissue / recommence the tender process. Any decision of the IIM-Calcutta in this regard shall be final, conclusive and binding on the agencies.
      7. If for any reason whatsoever, the empanelled agencies fails to provide the required vehicle as requisitioned, the same shall be treated as breach of contract on the part of the agency/agencies and under such circumstances, IIM Calcutta will have full right to make substitute arrangement at the risk and cost of the selected agency/agencies..
    17. IIM CALCUTTA RESERVES THE RIGHT TO TERMINATE THE CONTRACT IN CASEANY AGENCY BECOMES INSOLVENT OR IS CONVICTED IN A COURT OFLAW.
      If, at any time, the agency becomes insolvent or files an application for insolvency or anycreditor of his moves the court for adjudicating him as an insolvent or, if he is convicted in anyCourt of law, IIM-Calcutta will have the absolute option of terminating the contract forthwith and theAgency shall have no right for damage or compensation on this account.
    18. Notices etc. :

      All notices to be issued and action to be taken for and on behalf of IIM Calcutta shall be issued or taken on his behalf by the Officer-in-charge, Transport Department of the Institute. The agency shall furnish to IIM Calcutta the names, designation and addresses/e-mail of his authorized representatives, and all complaints, notices, communication and references shall be deemed to have been duly served to the agency if delivered to them or his authorized representative or left at or posted at the address so given.

    19. Award of Contract:
      1. A Letter of Award for empanelment shall be issued with schedule of rates corresponding to each vehicle for providing vehicle hiring services as per terms & conditions mentioned in the tender document. In the event of violation of any of the terms & conditions of the contract during the period of empanelment, the Security Deposit shall be forfeited. The decision of IIM-Calcutta shall be final and binding and no claims whatsoever shall be entertained in this regard.
      2. The selected agencies need to accept the offer of empanelment within 7 days from such communication from IIM-Calcutta, failing which offer will be withdrawn.
      3. IIM-Calcutta may at any time terminate the Contract by giving advance notice of two months in writing to the empanelled Bidder(s), without any compensation, if the Bidder becomes bankrupt or otherwise insolvent, provided that such termination will not prejudice or affect any right of action or remedy which has accrued or will accrue thereafter to IIM-Calcutta.
      4. If the empanelled bidder(s) are not able to fulfill their obligations as per the empanelment, IIM-Calcutta reserves the right to accomplish the work through alternate source at the cost and risk of the defaulting bidder.
    20. Applicable Law and Jurisdiction:

      All matters connected with this shall be governed by the Indian law both substantive and procedural, for the time being in force and shall be subject to the exclusive jurisdiction of Indian Courts at Kolkata only.

    21. Arbitration :

      All disputes regarding quality, specification, rates etc. shall be tried to be settled mutually my making references to conditions of contract documents or prevailing local practices etc., but if not settled mutually, shall be referred to arbitration. Such arbitration shall be governed by the provisions of the Indian Arbitration and Conciliation Act, 1996 or as amended from time to time. The venue for such arbitration will be in Kolkata. The award of the Arbitrator shall be final, conclusive and binding on all parties to the contract.

    Pre-qualification Criteria for providing & fixation of carpet in the Auditorium at IIM Calcutta

    IIM CALCUTTA invites sealed applications from intending Firms / Vendors in the prescribed Application form (Annexure – I) for short listing for the purpose of participation in the said work.

    Required criteria for such short-listing are furnished below:

    1. Should provide photocopy of the following statutory compliances certificates:
      1. Proper trade and professional license.
      2. Date of Incorporation Certificate.
      3. Company PAN Card.
      4. VAT / WBST registration Certificate.
      5. Income Tax Return Statement of last 3(three) years.
    2. Should have a business establishment in Kolkata.
    3. Should have experience and credentials for similar nature of work. (i.e. providing and fixation of Carpet in an auditorium in IITs/IIMs/Govt. Sectors/Corporate Bodies etc.) Supporting documents must be enclosed with the application.
    4. Operational experience for similar nature of work should not be less than 05 (Five) years.
    5. Audited balance Sheet of last 3(three) years. (i. e. 2012-13, 2013-14, 2014-15).
    6. The Annual turnover must be more than Rs.30, 00,000/-(Thirty lacs only). Supporting documents must be enclosed.
